Family Pre-Check Form

When you arrive to one of our campuses, you will check-in and receive name tags for your children. If this is your first time visiting Grandview (or you haven't checked in with us before), please take a few moments to complete this pre-check form to facilitate the check-in process. Grandview will link the name and email address you provide in this form with your Church Center account; if you have a Church Center account with Grandview, please use the name and email associated with your Church Center account.

Tell Us About Your Family

Grandview uses Church Center to help us create a safe and secure environment for our children. Tell us a little about your family. Adults and children added here will become part of your household (you can edit them later by using the Church Center app on your mobile device). Be sure to include important information such as medical conditions or special considerations.

Campus Preference

Grandview has two campuses, Buffalo and Cityview. Please let us know which campus is your preferred campus. This selection does not affect your attendance, it just helps us prepare for your visit.
